PASSPORTS PROGRAM
 
Passport is funded by the Ontario Government for the purpose of fostering independence, increasing participation in the community, promoting social inclusion, helping young people transition from school to the community and support families and caregivers. It is available to anyone who already qualifies for adult developmental services through the Ministry of Child, Community and Social services, including anyone 18 years of age and still in school. Passport Funds are able to be used for supports such as:
  • Community participation and Activities of Daily living
  • Paying a support worker to help with community participation and daily living activities
  • Caregiver respite
  • Person-directed planning that builds on the individual’s interests and strengths, and identifies the supports needed to help the individual achieve their goals, and
  • Administration of Passport funding (up to 10% of Passport allocation).
 
Individuals who receive Passport Funding have a right to choose how their funds will be administered. They may choose to self-administer their funding, have an agency do so on their behalf and deliver services, or use a combination of both service options. Additionally, individuals may choose to purchase services and supports from these providers:
  • Developmental services agencies
  • Private service and support providers
  • Adult education providers
  • Personal support workers
  • Neighbours, family members, and friends.
 
Individual’s wishing to apply for Passport Funding
are encouraged to contact their local DSO.
