SUPPORTED LIVING HOMES
The Residential Program is designed to support individuals with a developmental disability to live in a group setting and stay in their community. Our goal is to ensure that each individual feels at home in their residence and a valued member of their community. This is achieved through an approach that encourages and supports the individuals to live a life of their choosing.
With a commitment to using a Person-Centred approach, we work collaboratively with the individual, their family and their support network to identify goals and the steps needed to achieve them. As such, we are able to provide varying levels of support based on the individual’s needs.
Supports may include skill building and assistance in the following areas:
-
Personal care and hygiene
-
Building relationships within their community.
-
Household care and maintenance
-
Menu planning, grocery shopping and meal preparation
-
Monitoring, coordinating and advocating for health care needs
-
Budgeting, ODSP assistance, and additional funding assistance as needed
-
Accessing community services, opportunities and activities
